Unlocking Jaw Pain: TMJ Facts and Fixes

Temporomandibular Joint Disorder (TMJ) affects millions of people, often without them realizing the root cause of their discomfort. From jaw clicking to headaches, earaches, and even neck pain, TMJ disorder can impact daily life in unexpected ways. But what if relief was possible?

In this episode of Get Back to Your Life®, VSI Regenerative Medicine Director Dr. Niteesh Bharara and VSI Physical Therapist Diego Herrera break down TMJ pain—what causes it, how it progresses, and the treatments that can make a difference. We also hear from Caroline, a patient who found life-changing relief from chronic TMJ pain after years of struggling.

What is TMJ Disorder and How Does It Affect You?

The temporomandibular joint (TMJ) connects your jaw to your skull, allowing you to eat, speak, and express emotions. But when this joint malfunctions, it can cause persistent pain and a range of debilitating symptoms.

According to Dr. Bharara, “This joint is essential for almost everything we do—eating, speaking, even breathing. When it becomes dysfunctional, it’s not just about jaw pain anymore. We see patients with headaches, neck pain, dizziness, and even ear ringing.”

Many people dismiss TMJ pain as minor. If left untreated, however, it can lead to chronic discomfort, weight loss due to difficulty eating, and even social withdrawal. Physical therapist Diego Herrera highlights how impactful TMJ disorder can be: “I’ve had patients drop out of school or quit jobs because they couldn’t sit through a lecture or focus at work. It’s not just jaw pain—it’s a whole-body experience.”

Common Causes of TMJ Pain

TMJ disorder has multiple causes, and in most cases, it’s a combination of factors. Diego notes, “It’s rarely just one cause. TMJ pain is often the result of accumulated stressors over time.” These stressors include:

  • Bruxism (Teeth Grinding and Clenching): Often caused by stress, this repetitive motion puts excessive pressure on the jaw joint.
  • Poor Posture: A forward head position can strain jaw muscles, leading to misalignment and pain.
  • Joint Dysfunction: Issues like arthritis or a misaligned disc can lead to clicking, locking, and pain.
  • Dental Procedures: Keeping the mouth open for extended periods can contribute to TMJ issues.
  • Stress: Emotional tension can manifest in jaw clenching and muscle tightness.

How Do You Know If You Have TMJ Disorder?

A clicking jaw doesn’t always mean you have TMJ disorder. “Clicking alone isn’t always a problem,” Dr. Bharara explains. “But if pain accompanies the clicking or the jaw starts locking, it’s a sign something more serious is happening.”

Common symptoms of TMJ include:

  • Jaw pain or tenderness
  • Clicking or popping noises
  • Difficulty chewing
  • Headaches or migraines
  • Ear pain or fullness
  • Neck and shoulder tension

If these symptoms persist, seeking treatment early can prevent the condition from worsening and the ability to find relief through non-invasive methods.

Finding Relief from TMJ Pain

Many people assume they should see a dentist first for TMJ pain, but experts suggest otherwise. “Most dentists focus on dental misalignment,” Dr. Bharara explains, “but TMJ is a joint disorder, which means physical therapy is often the best first step.” At VSI, an interdisciplinary approach ensures patients receive the most effective treatment. Options include:

1. Physical Therapy

Herrera emphasizes, “The right exercises and education can completely change a patient’s outlook. We teach them how to break the pain cycle.”

2. Regenerative Medicine: PRP for TMJ

For patients with severe or chronic TMJ pain, Platelet-Rich Plasma (PRP) therapy can be a game-changer. Caroline, who suffered from debilitating TMJ pain for years, found hope through PRP therapy at VSI. “I was at my wit’s end. The pain was constant, and I had tried everything—mouth guards, Botox, dry needling. Nothing worked,” she recalls. “Dr. B looked at me and said, ‘You won’t be in pain forever.’ I didn’t believe him at first.”

PRP therapy involves drawing the patient’s blood, concentrating the healing factors, and injecting them into the TMJ joint. This promotes natural healing, reduces inflammation, and can restore joint function. Caroline’s transformation was profound: “After about three months, the sharp shooting pain was almost gone. At six months, I was singing again, eating normally, and living my life. Now, I have zero pain.”

3. Lifestyle Changes for TMJ

Small changes can make a big difference in TMJ pain management:

  • Avoid excessive chewing (gum, tough meats, crunchy foods).
  • Practice good posture to reduce strain.
  • Use heat or ice therapy to relax muscles.
  • Try relaxation techniques to minimize jaw clenching.
